In addition to both of you being present for the license issue, you must provide a valid picture identification. This can be your driver's license or passport.

Your birthplace, and the names of your parent will be documented on the license application.

If relevant, final divorce decree documents will be needed as well, as is written permission from a legal guardian or parent for those under 18 years of age.

There is no waiting period for your license and do not include blood tests and no vaccinations required. The license is valid for 30 days and the $50 (fee at the time of writing) must be paid in cash.

The central marriage license office is located in Oahu at the Hawaii State Department of Health.

THE STATE DEPARTMENT OF HEALTH
1250 Punchbowl Street • Honolulu
Monday through Friday, 8:00 am to 4:00 pm
Call (808) 586-4545

An appointment to obtain the license should be scheduled directly with a license agent. After the wedding, the officiant will send the license for filing as the official marriage certificate.

A certified copy is mailed to the couple approximately 120 days later.
